This issue happens because the Security Agent (SA) cannot properly replace the rating when the malicious website is accessed through a proxy server.
To resolve this, add the proxy server's process name in the Process Exception List of WFBS. 
  1. Open the Security Server console.
  2. Go to Preferences > Global Settings.
  3. Go to the Desktop/Server tab.
  4. Scroll down to find Web Reputation and URL Filtering section.
  5. Type the proxy server's process name in the field provided, and then click Add.
  6. Click the Save button at the bottom of the page.
